What is Surrogacy

Gestational surrogacy is the procedure whereby a woman – the gestational surrogate – consents with an embryo copy process, otherwise known as an IVF Procedure. During this technique, an embryo (or plural embryos) deriving from the couple obtaining the surrogacy service is implanted in the surrogate. If and when the procedure is successful, the surrogate becomes impregnated with the child of the couple (the planned parents). The amount of engagement with the surrogate thereafter is absolutely up to the intended parents – typically, the couple is associated with the initial IVF treatments, as well as being present at the moment of birth of their child. They then assume responsibility for the child post-delivery and make the long term change to legal parenthood.

What is IVF

IVF is a process women choose opt for when they are experiencing difficulties with infertility. The conventional IVF involves removing the human oocytes from the ovaries and the fertilization process, accomplished by “letting" motile spermatozoa to fertilize the oocytes into a particular culture medium. This means that once embryos are formed, they are moved into the uterine cavity through an activity called Embryo Transfer. IVF is employed to mimic the biological functions, in vivo.

IVF can be used for women who have blocked or broken fallopian tubes, thus stopping the egg and sperm from meeting naturally. IVF may also help you and your spouse if your infertility is unexplained and/or the male spouse has a low sperm count. Other factors like mature age endometriosis, immunology, obesity and psychology may lead people to infertility. IVF can be the option for you if these factors enter into play at this stressful time in your lives.

IVF Timeline

Step 1: Follicular Development and Monitoring

Throughout a woman"s normal menstrual period, LH and FSH are the hormones that the body uses to encourage development and maturation of a single oocyte. In order to optimize your pregnancy rates during IVF, it"s important to have more thatn one tmature oocyte available for fertilization, embryo development and transfer into the uterus. The ovaries are therefore activated using higher levels of the stimulating hormone that naturally exist within you. This is achieved using Gonadotrophin injections. Various stimulation regimes are present at the moment, and the doctor chooses the one best suited to your biological system, in order to achieve a great number of high quality oocytes. Close monitoring of the follicles (the sacs which contain the growing oocytes in the ovaries) is required and is achieved with regular ultra-sound in order to determine their size and their numbers . At the correct timing, an additional amount of hormones will be given, aiding with the ultimate maturation of the oocytes. Based on all the available responses and laboratory findings, the most suitable time for the recovery will be identified.

Step 2: Oocyte Recovery

This is the actual procedure by which the doctor collects the oocytes of the female body, a procedure needing anesthesia. A little needle enters the top part of the vagina, through the assistance of the ultrasound, into the ovary"s follicles. The liquid that is aspirated will then be studied in the lab, where the oocytes will be determined and moved into culture media inside an incubator at 37oC to continue the maturation process for several more hours.

Step 3: Fertilization of OOCYTS

In this step, your partner will have to give his test of sperm on the day of the oocyte recovery which will be maintained until needed). His sperm will be carefully made ready with a particular technique and the healthy spermatozoa separately. There are approximately 100,000 motile spermatozoa added per oocyte, with 4-6 hours after oocyte recovery. This is in order for the fertilization to take place within the following 12-18 hours in the incubator. The insemination process will lead to the fertilization of the oocytes, and ensuing embryos are transferred to different culture media to be viewed closely over the next 48 hours.

Step 4: Embryo Transfer

Top quality embryos are chosen and are moved through the womb using a small catheter on the basis of their rate of progress and morphology. The amount of embryos that"ll be transferred is decided according to the age and medical history of the potential mother; alongside the quantity and quality of the embryos which have been released during each therapy cycle. For each couple, there"s a possibility of having more than one child. Embryo transfer is a pain-free technique, much like having a cervical smear, and fortunately takes only a few minutes. Once the process has been completed, we recommend a brief resting period in the recovery room. One day after, oocyte retrieval progesterone and other supplements will be recommended to ensure that the lining of the uterus receives sufficient stimulation and sustains the implantation of the embryos. Quantitative pregnancy tests with blood must also be svheduled 12-14 days after embryo transfer.
