What"s Surrogacy

Gestational surrogacy is the procedure whereby a woman – the gestational surrogate – consents with an embryo transfer process, otherwise known as an IVF Procedure. In this treatment, an embryo (or plural embryos) deriving from the couple obtaining the surrogacy service is implanted in the surrogate. If and when the procedure is successful, the surrogate becomes impregnated with the child of the couple (the planned parents). The amount of engagement with the surrogate thereafter is absolutely up to the intended parents – typically, the couple is associated with the initial IVF treatments, and being present at the point of labor and birth of their child. They then assume responsibility for the child post-delivery and make the permanent move to legal parenthood.

What"s IVF

IVF is something women choose to undergo when they are experiencing problems with infertility. The traditional IVF involves the removal of the human oocytes from the ovaries and the fertilization process, achieved by “letting" motile spermatozoa to fertilize the oocytes into a special culture medium. This means that once embryos are created, they are moved in to the uterine cavity through a process called Embryo Transfer. IVF is employed to mimic the biological processes, in vivo.

IVF is used for women who"ve blocked or damaged fallopian tubes, thus stopping the egg and sperm from meeting naturally. IVF also may help you and your partner if your infertility is unexplained and/or the male partner has a minimal sperm count. Other factors like mature age endometriosis, immunology, obesity and mindset may lead a couple to infertility. IVF can be the treatment for you if these factors come into play during this stressful time in your lives.

IVF Timeline

Step 1: Follicular Development and Monitoring

During a woman"s normal menstrual cycle, LH and FSH are the hormones that your body uses to stimulate development and maturation of an individual oocyte. In order to optimize your pregnancy rates during IVF, it"s important to have more thatn one tmature oocyte available for fertilization, embryo development and transfer into the uterus. The ovaries are therefore activated using higher amounts of the stimulating hormone that naturally exist within you. This is achieved using Gonadotrophin injections. A variety of stimulation regimes can be found at the moment, and the doctor chooses the one best suited to your biological system, in order to achieve a great number of high quality oocytes. Close monitoring of the follicles (the sacs that contain the growing oocytes in the ovaries) is necessary and is achieved with frequent ultra-sound in order to find out their size and their quantities. At the appropriate timing, more hormones will be given, aiding with the ultimate maturation of the oocytes. Predicated by all the available responses and laboratory results, the most suitable time for the restoration will be identified.

Step 2: Oocyte Recovery

This is the actual procedure by which the doctor gathers the oocytes of the female body, a procedure demanding anesthesia. A small needle enters the upper part of the vagina, through the guidance of the ultrasound, into the ovary"s follicles. The substance that is aspirated will be taken to the lab, where the oocytes will be reviewed and moved into culture media in an incubator at 37oC to continue the maturation process for a few more hours.

Step 3: Fertilization of OOCYTS

In this step, your partner will have to give his sample of sperm on the day of the oocyte restoration which will be maintained until needed). His sperm will be carefully made ready with a special treatment and the healthy spermatozoa separately. There are around 100,000 motile spermatozoa added per oocyte, with 4-6 hours after oocyte restoration. This is in order for the fertilization to occur within the next 12-18 hours in the incubator. The insemination process will lead to the fertilization of the oocytes, and resulting embryos are transferred to different culture media to be viewed closely over the next 48 hours.

Step 4: Embryo Transfer

Good quality embryos are chosen and are moved through the neck of the womb using a small catheter on the basis of their rate of development and morphology. The amount of embryos that"ll be transferred is set according to the age and health background of the potential mother; alongside the quantity and quality of the embryos which have now been released during each therapy cycle. For each couple, there is a possibility of having more than one child. Embryo transfer is a painless treatment, much like having a cervical smear, and thankfully takes only a few minutes. Once the job has been completed, we highly recommend a short resting period in the recovery room. The day after, oocyte retrieval progesterone and other supplements will be recommended to ensure that the lining of the uterus receives sufficient activation and sustains the implantation of the embryos. Quantitative pregnancy tests with blood also needs to be done 12-14 days after embryo transfer.
