What is Surrogacy

Gestational surrogacy is the procedure whereby a woman – the gestational surrogate – consents to the embryo copy process, otherwise called an IVF Procedure. In this procedure, an embryo (or plural embryos) deriving from the couple obtaining the surrogacy service is implanted in the surrogate. If and when the procedure is successful, the surrogate becomes impregnated with the child of the couple (the supposed parents). The level of involvement with the surrogate thereafter is completely up to the intended parents – typically, the couple is associated with the initial IVF treatments, as well as being present at the moment of delivery of their child. They then take responsibility for the child post-delivery and make the permanent change to legal parenthood.

What is IVF

IVF is an option women choose to undergo when they are experiencing problems with infertility. The conventional IVF involves removing the human oocytes from the ovaries and the fertilization process, accomplished by “letting" motile spermatozoa to fertilize the oocytes into a special culture medium. This means that once embryos are created, they are moved into the uterine cavity through an activity called Embryo Transfer. IVF is employed to mimic the biological functions, in vivo.

IVF is used for women who"ve blocked or damaged fallopian tubes, thus preventing the egg and sperm from meeting naturally. IVF also may help you as well as your partner if your infertility is unexplained and/or the male partner has a minimal sperm count. Other factors like mature age endometriosis, immunology, obesity and psychology may lead people to infertility. IVF could be the treatment for you if these factors come into play during this stressful time in your lives.

IVF Timeline

Step 1: Follicular Development and Monitoring

Throughout a woman"s normal menstrual period, LH and FSH are the hormones that the body uses to stimulate development and maturation of an individual oocyte. To be able to optimize your pregnancy rates during IVF, it is important to severa tmature oocyte for fertilization, embryo development and transfer into the uterus. The ovaries are therefore stimulated using higher amounts of the stimulating hormone that naturally exist in your body. This is achieved using Gonadotrophin injections. A number of stimulation regimes exist at the moment, and the doctor chooses the one suitable to your biological system, to be able to achieve a good number of high quality oocytes. Close monitoring of the follicles (the sacs that contain the growing oocytes in the ovaries) is necessary and it is achieved with regular ultra-sound to be able to ascertain their size and their amounts. At the appropriate timing, more hormones will be given, aiding with the final maturation of the oocytes. Predicated by all the available reviews and laboratory findings, the best time for the recovery will be identified.

Step 2: Oocyte Recovery

This is the actual procedure by which the doctor gathers the oocytes of the female body, a procedure necessitating anesthesia. A little needle enters the upper portion of the vagina, through the guidance of the ultrasound, into the ovary"s follicles. The fluid that is aspirated will be studied in the laboratory, where the oocytes will be reviewed and moved into culture media in an incubator at 37oC to continue the maturation process for a few more hours.

Step 3: Fertilization of OOCYTS

In this step, your partner will have to give his sample of sperm on the day of the oocyte recovery which will be preserved until needed). His sperm will be carefully made ready with a special procedure and the healthy spermatozoa separately. There are about 100,000 motile spermatozoa added per oocyte, with 4-6 hours after oocyte recovery. This is for the fertilization to occur within the following 12-18 hours in the incubator. The insemination process will lead to the fertilization of the oocytes, and resulting embryos are transferred to different culture media to be observed closely over the next 48 hours.

Step 4: Embryo Transfer

Good quality embryos are chosen and are moved through the womb using a small catheter based on their rate of development and morphology. The number of embryos that will be transferred is decided in line with the age and health background of the potential mother; alongside the quantity and quality of the embryos which have been released during each therapy cycle. For each couple, there is a possibility of experiencing more than one child. Embryo transfer is a pain-free procedure, much like having a cervical smear, and fortunately takes just a few minutes. Once the task has been completed, we highly recommend a short resting period in the recovery room. The day after, oocyte retrieval progesterone and other supplements will be given to ensure that the lining of the uterus receives sufficient stimulation and sustains the implantation of the embryos. Quantitative pregnancy tests with blood also needs to be svheduled 12-14 days after embryo transfer.
